Our program is based on a multi-disciplinary rehabilitative approach to renal care. The program includes specialized care for the medically complex individual with acute/chronic renal failure of insufficiency. Goals are established for functional improvement and recovery under the direction of a primary care physician and a rounding Nephrologist. Our On-Site hemodialysis eliminates the need to travel to and from a dialysis center.
Bedside dialysis simplifies scheduling and causes less interruptions to the daily routine leaving more time for therapy sessions and family visits.
Residents typically dialyze five days a week, for approximately 2.5 hours at a time. This shorter, and more frequent, dialysis is gentler on the body, improves health and improves quality of life. More frequent dialysis will lower stress on the heart and provide better blood pressure control with fewer medications. Residents recover more quickly after treatment, have an improved appetite and fewer restrictions related to fluid intake and diet.
Our Respite Care Program provides a safe, supportive and supervised environment for your loved one. Our programs allow you and the caregiver time off to rejuvenate from the demands of caring for a loved one at home. Our comprehensive programs include:
- 24 hour nursing care
- Three nutritionally balanced meals and snacks daily
- Medication administration
- Assistance with activities of daily living including bathing, dressing, propelling, and meals
- Physical, occupational and speech therapy
- Recreational activities throughout the day
Our staff prepares an individual care plan for each resident. The care plan identifies goals and plans for each resident to become as independent as possible in the facility or in returning to a community setting. To facilitate our patients’ recovery at home, our team of qualified professionals will coordinate their needs with community agencies and services to ensure a smooth transition. Our case management staff will assist in setting up home health care or other services needed in your area. A well-coordinated discharge plan will enhance their success in the community.
Delicious and nutritionally balanced home cooked meals are provided. An assortment of substitute meals are always available upon request.
A full range of fun and stimulating activities are offered seven days a week to keep our residents active and engaged.
Our clinical team provides assessment and ongoing services for residents according to their unique needs, including medication management and supportive therapies. Residents are ensured that their medication is available and taken correctly.
Our Alzheimer’s care programs treat the unique needs of individuals with Alzheimer’s disease and Dementia or other related memory impairments. Our focus is to recognize each person’s physical, mental, social capacities and or limitations. We provide a secure environment where individuals can maintain an optimal level of independence, and an enhanced quality of life.
All Services are provided by our experienced staff members. We focus on individualized needs as directed by each resident’s primary physician. Everyone works together as a team to help accelerate recovery.
